Consent for Routine Health Services

Dear Parents and Guardians,
We need your immediate assistance in completing the annual health and mental health consent form for your child. In accordance with a new state law, Texas Senate Bill 12 (SB 12), all schools are required to obtain written parental consent before providing students with routine physical or mental health services.
Routine services include, but are not limited to:
  • First aid for minor injuries (like scrapes or headaches)
  • Vision and hearing screenings
  • Short-term, routine counseling or mental health check-ins
If we do not receive this completed form, our school nurses and counselors will be unable to provide these routine services to your child, except in a life-threatening emergency. This means if your child is ill or has a minor injury, we would need to contact you to pick them up before providing any care.
